Referring a Patient

Associates in ENT welcomes the privilege of working with you on the evaluation and care of your patients. We strive to offer your patients the most advanced and up-to-date services in a compassionate and supportive setting. We will also do our utmost to accommodate your patients in a timely manner and communicate with you through prompt consultations and clear communication.

You may schedule a consultation for your patient by submitting the provided referral form by fax to (423) 635-7544 or call the referral line at (423) 602-7000. Upon receiving the fax, we will schedule an appointment and communicate this to you by telephone. You may also call our office on our physicians' line at (423) 209-9114 and someone will help you. If your patient needs to be seen immediately, we will schedule an emergency visit on the same day.
